Athletic & Activity Participation Requirements
To ensure the health, safety, and success of all student-athletes, Forest Brook Middle School requires the following for participation in any athletic program:

Current Physical Exam: A completed and signed physical form from a licensed medical provider dated within the current school year.

Parent/Guardian Consent: A signed permission form allowing participation in school athletics.

Academic Eligibility: Students must maintain passing grades in all classes, meeting district and UIL eligibility standards.

Behavior Expectations: Student-athletes are expected to model good sportsmanship, respect, and discipline on and off the field.

Attendance: Regular school attendance is required to participate in practices and games.
